Device Return Form

Pursuant to Entrupy’s updated Terms of Service, Entrupy will no longer be offering hardware as a service, including the lease of any physical devices as well as any related peripherals (e.g., chargers) (“Hardware”) in connection with its authentication services. The authentication services previously provided to you (“Customer”) through Entrupy-supplied Hardware are now capable of being hosted on your own devices.

As part of this transition, Entrupy is offering each Customer two options with respect to each item of Hardware currently in your possession: (a) purchase the Hardware from Entrupy; or (b) return the Hardware to Entrupy.

You must indicate your election for each item of Hardware by completing either the Device Return Form or Device Purchase Form no later than June 30, 2026 (the "Election Deadline"). If Entrupy does not receive a completed Device Return Form or Device Purchase Form by the Election Deadline, Customer will be deemed to have elected to purchase all Hardware currently in Customer's possession that remains subject to this transition process, and Entrupy may assess and invoice Customer a purchase fee of one hundred ($100) U.S. dollars per item of Hardware without further notice.

1. Device Return

For any item of Hardware that you have elected to return, Entrupy will, at Entrupy’s expense, provide you with a prepaid shipping label and any associated return shipping instructions within a reasonable time following Entrupy’s receipt of this completed and submitted Form. You shall ship the Hardware within ten (10) business days (“Return Period”) after Entrupy delivers the shipping label.

2. Return Election: Hardware Condition

The following attestation applies to every item of Hardware identified for return in this Form. Good Condition Attestation. By checking the Good Condition Attestation checkbox below, you attest and certify to Entrupy that every item of Hardware that you have elected to return (as identified by the Hardware serial numbers submitted in this Form) is in Good Condition. For purposes of this Form, "Good Condition" means the Hardware is complete, functional, and free from damage (other than normal wear and tear commensurate with ordinary business use), with no missing components, and suitable for continued use by Entrupy. If, upon receipt and inspection, any returned item of Hardware is determined by Entrupy, in its sole discretion, not to be in Good Condition, Entrupy may deem such item of Hardware to have been purchased by Customer, in which case the one-time Purchase Fee of one hundred ($100) U.S. dollars per item of Hardware will be assessed and added to Customer's next invoice.

3. Customer Certification

By submitting this Form, the undersigned, on behalf of Customer, certifies that the information set forth in this Form (including all Hardware information and serial numbers provided) is true, complete, and accurate, and that the undersigned has full authority to make the elections and attestations stated in this Form on behalf of Customer. Customer further certifies that the Hardware serial numbers submitted in this Form accurately identify the Hardware subject to this return election.
